We have been made aware of some inaccurate information circulating regarding special ‘rescue packs’ for patients with pre-existing respiratory conditions such as asthma or chronic obstructive pulmonary disease (COPD).


Please do not contact your GP practice for a rescue pack. You should continue to manage your condition in the usual way and if you feel you have symptoms of COVID-19, go to https://111.nhs.uk/covid-19 before doing anything else.

You will note that there is no recommendation to have a rescue pack as there is no evidence for their use and indeed their are concerns steroid will reduce your immunity to fight off this virus.

New Patient Registration - NEW PATIENTS WELCOME!

nurse_senior_bpThe Boulevard welcomes everyone who moves into or lives in the practice area who wishes to register with us.

To register please call in to reception with your NHS NUMBER* and some official form of identification (passport, driving licence or birth certificate).  You will be required to complete a registration form (available at reception) and invited to attend a “Registration Medical” with our Health Care Assistant. 

This “medical” will only take about 20 minutes and ensures that the practice has all the relevant information regarding your health care needs.  Another form will be given to you to complete at home to get some details about your medical needs. This should be returned when you attend your medical appointment.

* We cannot register you without your NHS number (you can get this by contacting your previous GP surgery)

Preference of doctor

Although patients are registered with the “Practice” and not an individual doctor, patients can request to see a regular doctor.  Please advise a receptionist if you wish to have a preferred doctor recorded.  Whenever you require an appointment the surgery will try and provide this with your preferred doctor, however this may not always be possible.
